Introduction

Fever is one of the most common symptoms people experience. And for many, especially in African settings, the first thought is simple:

“It’s probably malaria.”

So treatment begins.

But sometimes, it does not work.

Because fever is not a diagnosis. It is a signal.


What Is a Fever

A fever is the body’s response to infection or inflammation.

Normal temperature ranges between 36.5 and 37.5°C.
Fever is usually above 38°C.

What matters most is the cause.


Common Causes of Fever

Infections

  • Malaria
  • Typhoid
  • Pneumonia
  • Urinary tract infections
  • Viral infections

Other Causes

  • Inflammation
  • Drug reactions
  • Sepsis

Not every fever is malaria.


Why Self Diagnosis Can Be Dangerous

From clinical experience, many patients:

  • Assume fever equals malaria
  • Take antimalarials without testing
  • Use antibiotics unnecessarily
  • Mask symptoms with painkillers

This delays proper treatment.

By the time care is sought, the illness may be worse.


When Fever Is a Warning Sign

Seek care if you have:

  • Fever lasting more than 2 to 3 days
  • Difficulty breathing
  • Confusion
  • Severe weakness
  • Reduced urine output

These may indicate serious illness, including sepsis.


The Link Between Fever and Sepsis

Sepsis often starts as a simple infection.

It may look mild at first.

But it can progress quickly if untreated.


What You Should Do

✅ Do:

  • Monitor temperature
  • Stay hydrated
  • Seek medical care if symptoms persist

❌ Avoid:

  • Self-prescribing antibiotics
  • Taking antimalarials without confirmation
  • Ignoring worsening symptoms

Patients often present after self-treating for malaria, only to be diagnosed with another condition.

Delays increase complications.
